How to complete Activity and Assessment forms for Groups

Filling out forms for Groups is very similar to filling them out for an individual person, but with the extra ability of selecting participants. 

First, find the Group you want to add forms to by using the Jump To Contact search bar.

Once on the Group's Dashboard, make sure the group is enrolled in the correct program for the form you want to fill out. If not, enroll them through the Programs tab.

If you would like to complete an Activity, or service, form for the group, click on the Activities tab.  Otherwise, click the Assessments tab. Select the forms you want from the dropdown menu.

Once you are on the form page, you will see a blue Participants box. Here, you can specify which group members participating in this activity/assessment, Toggle All participants, or leave participants blank.

If the group has more than 25 members, you will see a different way to attach participants. Instead of checkboxes for each member of the group, you'll have the ability to enter names to narrow the list and select them from there.

Complete the fields in the form, then press Save. Once you save, it will appear on the Groups Assessments tab, and also on each participating Group Member's Assessments Tab.

If you have a group form that does not need any Participants selected, your admin can choose to hide this box from the form in the Form Builder settings. Learn how to Hide Group Participants here!
