How to pull dates associated with form results

You may need to know what date a form was completed or when a service was administered. Pulling this information in the Report Builder is easy - just follow the steps outlined below.

1. Open up the Report Builder and add any demographic data needed first.

  • If you need to display a contact's Unique ID, name, or any other personal information associated with the form, pull this information by opening up the Person or Group branch in the lefthand sidebar and checking the fields to add to the Report Builder

2. Find the name of the Activity or Assessment form that you would like to pull date information from.

  • Open up the Activity and Assessment branches and scroll down the alphabetical list of form names until you find your form

3. Choose from Date of Activity, Created time, or Last updated time.

  • Open up the form's branch by clicking on the + button. Here you will find different options for getting a date associated with the form

Date of Activity: This is the date that a staff member enters at the top of a form when starting a new form result. This date usually indicates the day that the service was performed. 

Created time: This is the date that the form result was saved to the VerticalChange system. For example - a staff member could perform a service on 2/10/2017 (the Date of Activity), but if they don't fill out the form on VerticalChange until a week later, the Created time will be listed as 2/17/2017. 

Last updated time: This is the date that a form result was last updated. This date may differ from the Created time if a user went back to the form to edit it after it had already been saved. 

  • Note: While the field is named 'Created time' and 'Last updated time', these will only display dates in the form of MM/DD/YYYY in the Report Builder.

4. Once you have added your date field to the Report Builder, you can set filtering options on your dates, save your report or export to a CSV.
