How to assign a form to a person, group, or program

In Vertical Change, forms can be completed at 3 levels.

Individual (Person)
Group
Program

When you attach a form to an individual, you can only complete a form at the individual, or "person" level.  Same goes for Group and Program.  Navigate to a group contact, say, a class you've created or a family unit with individuals attached.  You can complete the form only from that group profile.  

How to assign a form to a contact?

Navigate to the form you want to assign, go into Edit mode, and select which overall contact type to scope the form to.

How to restrict a form to one or more contact types?

If you would like to restrict the form further to only show for a particular contact type, you can do so using the Restrict to Contact Type option. 

When you select People, Groups, or Programs from the Restrict to setting, the Restrict to Contact Type field will populate all of the different contact types available under that type of person (ex: People -> Child, Adult; Groups -> Family, Classroom, Site). You can select one or more type from the list to restrict the form to.

For example - you can restrict a form to only be available on Site contacts by selecting:
Restrict to: Groups
Restrict to Contact Type: Site

Or, you can restrict a form to only be available for your adult and children clients by selecting:
Restrict to: People
Restrict to Contact Type: Adult, Child

Note: If you leave the "Restrict to Contact Type" field blank, the form will show for all contacts under the category that is selected in the Restrict to field.
