Sometimes you need to skim data (without opening up every form result) from a long list of completed forms, or often you need to see at-a-glance a specific variable found on your form.  For example,  an assessment that gives a "total score" that determines the level of care a client needs.  Seeing that score immediately when opening your client's profile, will help optimize your time when managing your client's needs.

Here's how to do it.

** Available only for admin level users

  1. Navigate to FORMS on the left hand sidebar

  2. Find the form you would like to enable this feature on.

  3. Click on it and go into Edit mode by clicking the Edit button in the top right.

  4. Once you're inside the form scroll to the "Elect one or more fields for top level display" box.

  5. Click in the box and select one or more field names which you'd like to display on your contacts' list of completed forms.

  6. Save

Note: The first field in the form that you enable for top level display will show up in the form results on the Contact's Activities or Assessments tab. All of the fields you enable for top level display will show on the Contact dashboard if you have a Program Goal enabled for that form. (For more information on Program Goals, see Create and Edit Program Goals).
